Contact your health center and ask for access to the Patient Portal. Upon successful activation you will receive an welcome email with instructions on setting up your account.
Both tools will give you access to communicate with your doctor and care team, securely and safely view health records and information, update personal data, access your appointments, lab results and vitals, request medications refills, and participate in virtual visits.
The Patient Portal and Healow app have functionalities that allow you to reset your username and/or password. You can also call the front desk and we can reset the username and/or password for you.
One of the consents you must agree to is our vendor consent. My Health Information (patient portal) is powered by a vendor called eClinicalWeb, LLC (“ECW”). This consent outlines the Terms of Use Agreement (“Terms of Use” or “Agreement”) defines the terms and conditions under which ECW makes the Site available. The second is a medical center practice consent: addressing issues and policies of the practice in exchanging information with patients electronically. It is important to have your permission to communicate with you via The Health Portal, it’s another way to protect your patient information.

El Proyecto’s Behavioral Health Program provides counseling and referrals to men and women with substance abuse and mental health problems, including specialized services for women who are pregnant, postpartum or parenting. The Program is staffed by bilingual and drug-certified counselors. It assists men and women in areas including domestic violence/sexual abuse, self-esteem, peer support, life skills, health education, drug education, 12 step/relapse, and parenting who receive treatment rather than incarceration.
